If you are a designated provider within the LIFE Geisinger network, or a physician with a
frail elderly patient, we hope you will consider referring to LIFE Geisinger for dependent
care. We offer all-inclusive services to enable the older
adult to maintain their quality of life while living in their
own home. The LIFE Geisinger team coordinates and/or provides all
needed health care, including primary care and
specialist physician services (excluding emergency
services). These services extend to hospitalization and
home care when necessary. Designated as an Excellence in Care Dementia Program of Distinction by the Alzheimer’s Foundation of America, the LIFE Geisinger team is recognized as a national leader in caring for individuals with dementia disorders such as Alzheimer’s.

Pennsylvania are open to eligible residents in the
